What Exactly Is Masseter Botox?

The masseter is one of the primary muscles involved in chewing. Some people naturally have larger masseter muscles. Others develop more prominent masseters from years of clenching or grinding their teeth. Think about what happens when you repeatedly work a muscle at the gym. It can become larger and stronger. Your masseters can essentially do the same thing.
Botulinum toxin can be injected into the masseter muscles to temporarily reduce some of that muscular activity. Research has shown that botulinum toxin type A can reduce masseter muscle thickness and prominence in people with masseter hypertrophy. Studies have also investigated its use in people with bruxism and jaw clenching.
At Charmed Medispa, we are always looking at the person in front of us rather than simply treating a complaint.

“Sometimes a patient comes in telling me she wants a slimmer jawline, but when I have her clench, I realize just how strong those masseter muscles are. I am never interested in simply injecting something because we can.”
And that is an important distinction.
Can Botox in the Masseters Slim Your Face?
It can in the appropriate patient.
If a wider or more square appearing lower face is partly caused by enlarged masseter muscles, reducing the activity of those muscles can gradually decrease their prominence. That may create a softer, narrower appearance through the lower face.
A 2026 systematic review and meta analysis of randomized controlled trials found that botulinum toxin A improved masseter prominence and reduced masseter muscle thickness compared with placebo.
But here comes the Charmed common sense portion of today’s program.
Masseter Botox does not magically remove jowls, tighten loose skin, dissolve fat, or rebuild a jawline.
If your lower face looks heavy because of skin laxity, volume loss, bone structure or fat rather than a large masseter muscle, shrinking the masseter may not give you the result you are imagining. In certain faces, reducing too much masseter volume can actually make existing laxity more noticeable.
That is exactly why Googling “best Botox near me” should only be the beginning of the process. The assessment matters just as much as the injection.
What About Botox for Jaw Clenching and Teeth Grinding?
This is actually how many patients discover masseter Botox.
They may wake up with a tired or sore jaw. Their dentist tells them they are grinding their teeth. They have worn a night guard for years. Or their spouse politely mentions that they sound like they are chewing gravel in their sleep.
Clinical studies have found reductions in masseter thickness and improvements in some bruxism related symptoms following botulinum toxin injections, although results vary and the treatment is not a cure for the underlying cause of bruxism.
Some dentists recommend a mouth guard but patients report that they still clench down and still experience tension and pain. The mouth guard only protects their teeth from grinding down but doesn’t stop the masseter muscle from working.
One of the comments we commonly hear from patients after treating very active masseters is something along the lines of:
“I didn’t realize how hard I was clenching until my jaw finally felt like it could relax.”
That is the kind of difference patients sometimes notice before they ever notice a change in the mirror.
When Will I See My Jawline Change?
This isn’t typically an overnight transformation.
The muscular activity begins changing first. If facial slimming is one of the goals, the visible contour change occurs more gradually as the masseter becomes less prominent.
That is why we tell patients not to stare at themselves every morning thinking, “Is my face skinny yet?”
Give your body some time.
Studies evaluating masseter treatment have demonstrated reductions in muscle thickness over the following weeks and months, with effects diminishing over time as neuromuscular activity returns.
And just like Botox in other areas, masseter treatment is temporary.
How Many Units of Botox Do You Need in the Masseters?
This is one of the most common questions we get, and the answer is not a cute little universal number.
It depends.
Masseter size, strength, facial anatomy, asymmetry, treatment goals, previous treatment and the particular botulinum toxin product being used all matter.
Most importantly, Botox units, Dysport units and units of other neuromodulators are not interchangeable.
There isn’t one scientifically established dose that is perfect for every patient. In fact, the recent systematic review of randomized trials specifically concluded that available evidence is not sufficient to establish an optimal formulation adjusted dosing protocol.
Loraine puts it more simply:
“I don’t treat a number. I treat the muscle that’s sitting in front of me. Two people can walk into my office asking for the exact same thing and need completely different treatment plans. I would say however that the average dosage is 25 units of Botox per side and results usually last 4-6 months.”

Can Masseter Botox Have Side Effects?

Possible effects can include temporary chewing fatigue or weakness, changes in the smile, asymmetry and unwanted changes in facial contour. Published research has documented temporary masticatory fatigue and smile changes following masseter treatment.
